that depends on your internet browser, usually you pick the folder manually in a dialog after you click the cursor.

Icon (.ico) contains all images visible in the editor. You can add or remove images for example using the the 'Icon' menu.

Yes, it is possible, but only from the listing of sets (here in your case). There, you may click an Edit link inside the box with your set.
